It constitutes a social measure designed to provide specific home care services to individuals and families, who, due to illness, disability or other, are unable to satisfy their basic needs and carry out their daily activities.

This response is offered to the residents of Carcavelos.

Assistance in outdoor activities and shopping for foodstuffs and other goods;
Provision of technical aids (articulated beds, wheelchairs, walkers, etc.)

Long-Term Care

The RNCCT (Rede Nacional de Cuidados Continuados Integrados – National Network of Long-Term Integrated Care) is an instrument created in 2006 (Decree Law no 101/2006) by the Ministries of Health, Labour and Social Security. It aims to provide hospitalisation to people with disabilities, especially the elderly, and/or long-term integrated health care along with social care offered at home. This context dictates the emergence of long-term care provided in conjunction with social institutions, as is the case of the Community Centre and the Centro de Saúde da Parede (Health Care Centre of Parede).

What is it?

It consists of the provision of specific care at home, when due to illness, disability or otherwise if not possible to temporarily or provisionally guarantee the satisfaction of the basic necessities of daily life.

Who can benefit from the Services?

All persons residing in Carcavelos who are in a situation of dependency.

How should I proceed to receive Home Care Support?

You should contact by telephone, e-mail or in person the JPAC, referring to the subject “Home Care Support”

When will you give me an answer?

Usually in 48h. If this time does not happen, you should make new contact with the Institution.

What kind of support can I use?

Provision of personal hygiene care and comfort; housekeeping and small cleanings at home; confection, transportation and distribution of lunches; accompaniment in Psychology; accompaniment in Physiotherapy; rental of Technical Aids; other services according to the specific needs of each case.

In a period from Monday to Sunday including Holidays, from 8 am to 5 pm, depending on the needs and availability of the Institution’s response.

What are my obligations?

Collaborate with the Team of Direct Action Assistants in the measure of their interests and possibilities, treating the elements of the Institution with respect and dignity; provide proof of income annually; to pay for the services provided; to comply with the Internal Regulation of Home Care Support.

How can I cancel services?

Formally must be communicated in writing, 30 days in advance.

However, given the multiplicity of circumstances, you can communicate the cancellation of services in person, by phone or email as early as possible.
